Step 1

Use the notes you took when completing the evaluation. Mention, in the
comments part of the attendance section on the evaluation, any problems that
you found.
Step 5
Tell the employee of any attendance problems, if you go over the evaluation
in person. Ask how he plans to improve his attendance.
